At Distinctive Home Physical Therapy we accept a wide range of insurances. Most of our senior patients are fully covered by Medicare. Secondary insurances such as Empire Blue Cross Blue Shield, Well-Choice, AARP, and United will cover the copayment.

Do I need a physical Therapy prescription?

Medicare patients require a prescription from their MD before the initial evaluation. This can easily be done by making a quick phone call to your doctor. You can also have a doctor fill out our Physical Therapy prescription form. All Direct Access patients must have a prescription if treatment last longer than one month.

Go here and print out the schedule forms. If you need to, call our office — (516) 214-8307 — and we'll mail or fax the forms to you.

Step 2
Complete the forms and return them to us along with a copy of your primary and secondary (if you have one) insurance cards, along with your physical therapy prescription. You can mail it, fax it or email it!
